We are proud to be an In-Network provider for most major vision insurance plans. When you visit us, we handle the paperwork for you, and your benefits are applied directly at the time of purchase.

Out of Network: VSP
While Obsidian is an “Open Access” provider for VSP, we are not technically in-network. However, this does not mean you can’t use your benefits here. In fact, most VSP plans include an “Out-of-Network” allowance that can be applied to our frames and lenses.

How to Process Your VSP Claim
We make the reimbursement process simple so you can still enjoy our curated collection:
- Pay at Time of Service: You will pay for your eyewear at our boutique. We will provide you with a detailed, itemized receipt specifically formatted for insurance claims.
- Submit Your Claim: Log in to your VSP Member Account and navigate to the “Claims & Reimbursement” section.
- Upload & Receive: Upload a photo of your receipt and submit the form. VSP typically processes these claims within 10 business days and will mail a reimbursement check directly to your home.
- You may also print the out of network reimbursement form below and mail it to VSP Member Claims; PO Box 495933; Cincinatti, OH 45249.
As an independent provider, Obsidian Exceptional Eyewear is not directly affiliated with VSP. Because we do not have access to your private benefit portal, we are unable to verify eligibility or guarantee specific reimbursement amounts at the time of purchase. To ensure you can shop with total confidence, we recommend contacting VSP directly to confirm your out-of-network benefits before your visit.
Why is Obsidian an out-of-network provider for VSP?
Obsidian is 100% optician-owned and operated. VSP’s standard in-network contracts generally require that a practice be at least 51% owned by an Optometrist. Because we have chosen to remain an independent boutique led by master opticians rather than a medical corporation, we are classified as an “Open Access” provider.
What are the benefits of an optician-owned boutique?
Our independence is our greatest asset. By remaining optician-owned, we aren’t bound by insurance-mandated restrictions on which frames we carry or which laboratories we use. This allows us to focus entirely on the art of the fit and the quality of the lens, sourcing handcrafted frames from global artisans that you won’t find in a typical “in-network” office.
Does this mean I can’t use my VSP benefits at Obsidian?
Not at all. Most VSP plans include an Out-of-Network Allowance. You can still enjoy the Obsidian experience, choose the perfect frames, and pay for your eyewear at the time of your visit. We will then provide you with a specialized, itemized receipt to submit to VSP for your reimbursement.
Will I save more money by staying in-network?
While in-network providers may have lower initial out-of-pocket costs, they are often restricted to using insurance-owned labs. At Obsidian, we utilize premium independent laboratories and world-class lens technologies. Many of our clients find that the superior clarity, durability, and personalized styling we offer provide a much higher value for their investment.
How do I find out my reimbursement amount?
Since VSP keeps your plan details private, we recommend calling them directly or logging into your VSP member portal to check your “Out-of-Network Schedule.” Every plan is different, but most offer a significant allowance for frames and a reimbursement schedule for lenses.
We want the reimbursement process to be as simple as possible. Below, you’ll find a library of forms for our accepted and non-network insurance providers, should you need them. While we can help you identify which form you need, we recommend a quick call to your carrier to confirm your specific plan’s reimbursement rates, as we may not have access to your private policy details.
